Camp Massasoit & Spring Explorers

- Emergency Contact/Health Record Form - Must be completed
- This form must be filled out. If you are submitting your immunization and proof of physical independently of this form, the front side of this form must still be completed and submitted.
- Medication Authorization Form - Only if Needed
- Only required for campers bringing medication on-site to Camp Massasoit or Spring Explorers. This must be updated and resubmitted each year.
